In order to participate in Maui Goat Yoga events or classes, you must agree to the following Liability Release and Appearance Release agreements:
Liability Release
This agreement releases Wendy Hatchie, Andrew Hatchie, Keakoa Aukai Aina Farms and Community Healing Farms, A Veteran Project / DBA Maui Goat Yoga and all those related to Maui Goat Yoga from all liability relating to injuries that may occur during Maui Goat Yoga Events or Classes.
I agree to hold Wendy Hatchie, Andrew Hatchie, Keakoa Aukai Aina Farms and Community Healing Farms, A Veteran Project / DBA Maui Goat Yoga and all those associated to Maui Goat Yoga entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.
I also acknowledge the risks involved in Maui Goat Yoga.
I understand and swear that I am participating voluntarily, and that all risks have been made clear to me.
I swear I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.
I forfeit all right to bring a law suit against Wendy Hatchie, Andrew Hatchie, Keakoa Aukai Aina Farms, and Community Healing Farms, A Veteran Project / DBA Maui Goat Yoga and all those associated to Maui Goat Yoga for any reason.
Appearance Release
hereby acknowledge that I am of legal age.
I hereby acknowledge and give Community Healing Farms, A Veteran Project / DBA Maui Goat Yoga the right and permission to use my name and to use, reproduce, edit, exhibit, project, display, copyright, publish images and/or moving pictures and/or videotaped images of me with or without my voice, or in which I may be included in whole or in part, photographed, taped, videotaped, and/or recorded to circulate the same in all forms and media for art and advertising.
I hereby waive any right that I may have to inspect and/or approve the finished product or products or the editorial, advertising that may be used in connection therewith and any right that I may have to control the use to which said product may be applied.
I also acknowledge that I will sign a waiver of Release of Liability and Appearance Release at the time of signing up for an event or class.
